Affiliate Plan (A)	I am interested in building my business and would like to become a funeral home affiliate for Funeral-Packages.com. I understand and agree to pay a non refundable administrative setup fee of only $89.95.
Affiliate Plan (B)	I do wish to purchase a web site for my funeral home and become a funeral home affiliate. I understand and agree to pay $399.00 for a 4 page web site. I also understand and agree to pay a non refundable administrative setup fee of only $89.95.
Affiliate Plan (C)	Yes, I want to become a funeral home affiliate but at this time I do not wish to pay the $89.95 setup fee. I understand that by not paying a setup fee now, I agree to pay higher marketing fees for funerals I receive from Funeral-packages.com. I also understand that I can be permanently bumped from the rotation when the city or cities I service receive 3 funeral home affiliates who do pay the setup fee.
Affiliate Plan (D)	Yes, I am interested in becoming a Funeral Home Affiliate and would also like to purchase the exclusive rights for the cities I service. I understand and agree to pay a non refundable administrative setup fee of only $89.95. I also agree to pay a yearly fee of $125.00 for each city I choose to have the exclusive rights for (some restrictions apply).

The Funeral-packages.com website is free to the consumer. The marketing fee is calculated on a sliding scale as illustrated below.

If you decide on plan A, B or D, as described above, each Funeral Home Affiliate will be billed a marketing fee for each funeral package that is generated by the Funeral-Packages.com website.

Our sliding scale marketing fee is based on the funeral package price. Funeral Homes Affiliates will be billed this marketing fee based on the funeral package price. Marketing fees range from 2 percent to 4.5 percent of the funeral package price.

The following schedule illustrates how Funeral-Packages.com calculates the marketing fee charged to funeral home affiliates:

Funeral package price of $0.00 to $2,000.00 = $59.95
Funeral package price of $2,001.00 to $3,000.00 = $89.95
Funeral package price of $3,001.00 to $4,000.00 = $109.95
Funeral package price of $4,001.00 to $5,000.00 = $129.95
Funeral package price of $5,001.00 and Above = $159.95

If you decide to go with Plan C, the above marketing fees will be billed to you at 1.5 times the current rate.
